Medical form, lets make this easy. You need to fill out new forms by the Court of Honor.

Posted by michaeltneff on Sep 8 2018 - 7:53am

Troop, lets make medical forms easy.    The troop needs new forms from each scout, specifically part A&B, filled out by the September court of honor.  This will cover all the campouts during the school year.   Parts A&B are filled out by a parent.  No doctor needed for part A&B.  This way, the whole year, the troop is ready for all activities.  Here is the link to the form: https://www.scouting.org/health-and-safety/ahmr/
